From First Visit to Retainer

Most people weighing braces have a clear picture of the outcome and no picture at all of the sequence. They know what they want their teeth to look like at the end. What happens between now and then is a blank, and that blank is where most of the hesitation lives.

So here is the whole thing, start to finish, in the order it actually happens.

The Consultation

An exam, digital X-rays, photographs and a scan of the teeth. We look at how the bite fits together, not only how the front teeth line up – crowding, overbite, crossbite and open bite are different problems with different solutions, and two people with identically crooked front teeth can need completely different plans underneath.

You leave with the options that genuinely apply and a written fee. Nothing begins that day. Nobody should commit to eighteen months of treatment in the same appointment they first heard it described.

What the Records Actually Show Us

The imaging is not a formality. A panoramic X-ray shows every tooth present, including the ones still under the gum, and it catches the problems you cannot see in a mirror: an impacted canine sitting sideways, a permanent tooth missing entirely, a root angled into its neighbour. It also shows the wisdom teeth and whether they will matter.

The scan replaces the old putty impressions and produces a digital model of the bite we can measure precisely. Measuring is the point – deciding whether crowding is three millimetres or seven is the difference between aligning the teeth as they are and needing to create space first.

Placement Day

An hour to an hour and a half. Teeth are cleaned and prepared, brackets bonded individually, and the first wire placed. It does not hurt – there is no drilling and no injection, and the most common complaint is that holding your mouth open gets tiring.

Soreness arrives the next day and fades within three or four. Soft food for the first week is the practical advice: pasta, eggs, soup, yogurt. Over-the-counter pain relief and warm salt-water rinses handle the rest. The inside of the lips takes a week or so to toughen up, and wax over the brackets bridges that week.

The Visit Rhythm

Adjustments every six to eight weeks, fifteen to thirty minutes each, with a day or two of pressure afterwards. Between visits the wire works on its own – continuously, whether you think about it or not.

For patients balancing a seasonal schedule out east, appointments can usually be planned well in advance, and siblings can generally be grouped into the same visit. Ask about that at the consultation rather than assuming it, because practices differ.

Living With Braces Day to Day

Two things do most of the work: cleaning and not breaking anything.

Cleaning takes longer than it used to, and that is not negotiable – brackets give plaque places to sit, and plaque left around a bracket for months leaves permanent white marks on the enamel even after the braces come off. Brush after every meal, use a floss threader or a water flosser nightly, and keep your regular dental cleanings on schedule throughout treatment.

Breakage is mostly about food. Hard and sticky are the two categories that cause trouble: ice, nuts, hard candy, popcorn kernels, bagels, caramel, taffy. Crunchy raw vegetables and apples are fine cut into pieces. A broken bracket is not an emergency, but each one costs an unscheduled visit and a little momentum.

Elastics, and Why They Matter More Than You Think

If your plan involves bite correction, at some point you will be given elastics to hook between the upper and lower braces. They are the part patients quietly skip, and skipping them stalls bite correction completely – the wires keep straightening teeth while the bite relationship stays exactly where it started.

Full-time wear means full-time, minus eating and brushing. Patients who wear them as prescribed finish on estimate. Patients who wear them at night only add months, and they are usually surprised to be told the delay was avoidable.

Sports

Braces and contact sports coexist fine with an orthodontic mouthguard – the type designed to fit over brackets rather than the boil-and-bite kind, which moulds around the appliance and can pull on it. Wear it for practices as well as games. A blow to the mouth without one is how a bracket becomes a laceration.

What Actually Shortens Treatment

Keeping every appointment, wearing elastics exactly as prescribed, and not breaking brackets. Those three account for most of the gap between a case that finishes on estimate and one that runs six months over.

Notice what is not on that list: any device or technique that promises to accelerate tooth movement. Bone remodels at the speed it remodels.

Removal and Retention

Taking braces off takes under an hour and is painless. A retainer follows immediately – usually the same day, because teeth are at their most mobile in the first weeks after the appliances come off.

Retention is not optional. Teeth drift lifelong, and the retainer protects the result you paid for. Most patients get a clear retainer, a fixed wire behind the lower front teeth, or both. Full-time wear at first, then nightly, then nightly indefinitely. Everyone who tells you their teeth moved back after braces is describing a retainer they stopped wearing.

What Parents Ask Most Often

Whether their child can play sports: yes, with an orthodontic mouthguard. Whether treatment will affect schoolwork or speech: no, beyond the first few days. Whether the appliances will interfere with music: brass and wind players adapt within a few weeks, and it is worth mentioning at the consultation so the plan can account for it.

And whether waiting a year would be better. Sometimes it genuinely would, and we will say so – the efficient window for comprehensive treatment is once most permanent teeth have arrived. Starting early for the sake of starting means a longer course of care with no better final result.
